Player MUST have the completed paperwork PRIOR to being allowed to begin practice:
- Sports Physical Form
- Cardiac Arrest Form
- Concussion Form
- Consent to Play
- Player Behavior Contracts
- Player Contact Information Sheet
Players MUST have soccer cleats and shin guards to participate in practice.
