Please make sure you complete a health history form for your child at the beginning of each school year.

Students must be fever free for 24 hours without the aid of medication before returning to school after an illness, vomiting, or diarrhea.

The school must have a signed consent letter from your child's Doctor before the school can give Prescription Medication to your child. The school must have a signed consent letter from the child's legal guardian to give the child over the counter medications, such as ibuprofen.
