ATTENTION: New physical exams are required starting this year for each grade. Please choose the appropriate form for the upcoming grade for your child.
MUST BE DATED AFTER April 15, 2011 to be valid for the 2011-2012 Athletic Season, no matter what sport you are playing! This includes Cheerleading!
